If you are missing any documents, we will contact you by mail, email, or phone.

If you requested that we send you any documents, please be on the lookout for them by mail or email.

At the bottom of this page, you can download a copy of the application that has been submitted. A copy of our Notice of Privacy Practices has been sent to your email.

Once approved for the program, you will receive a letter in the mail with your approval date.
